I agree to the conditions and terms of those presenting this event including SkyQuest Outdoors and Camp Appanoose. I will abide by the regulations and requirements of the same, and conduct myself in an honorable way. I will uphold and abide by all the laws of the state of Iowa. A signed "Hold Harmless Agreement" is required before hunting. The total cost of the hunt is $450.00. My down payment of $225.00 is enclosed with this application. (You may also use PayPal, using skyquest@iowatelecom.net as the account.) You may elect to pay the full amount at this time. Any balance is due on arrival at Camp Appanoose on April 12, 2009, and is non-refundable at the sole discretion of SkyQuest Outdoors and/or Camp Appanoose.
